[REDACTED] was assisting [REDACTED] and [REDACTED] with the screw removal. Removing the locking hub assembly.

[REDACTED] was holding the screw extractor as [REDACTED] was turning the extractor to remove it. The extractor came free of the threaded mount and dropped down. [REDACTED] was not expecting this to happen and it was heavier than expected. His finger was caught between the extractor and the edge of the machine cover when it was hit smashed.
