[REDACTED] was working on moving the line 2 extrusion head to a cart for reassembly. Head was suspended on the end of a forklift fork about 5 1 2 feet above the ground. [REDACTED] was operating the forklift and [REDACTED] was assisting from the opposite side of the [REDACTED] keeping hoses from getting tangled.

[REDACTED] was trying to place a block between the extruder head and the cart and while moving around to position the block hit his head on the forklift fork. (One fork suspended the head the other fork was not holding anything)
