traversing through overhead door on a maintenance scooter

While associate was driving scooter through open overhead door the door came down on him. When the door came down it pinned him down by his face with his back across the seat frame. The sensor did not stop the door from coming down nor did the door immediately go back up after sensing an obstruction like it 's made to do. After being pinned several seconds the door went back up and maintenance manager and safety coordinator came to his aid.
