What happened
The team member was taking the bolts out of the holding furnace. One of the bars welded to the bolt head snapped off. Team member was trying to spin the bolt with his hand to be able to get a wrench on it and at the same time he triggered the impact on the nut. This caused his index finger on the bolt to get sucked in and smashed between the bar on the bolt and the holding furnace. Team member had to tighten the nut back down to free up his finger.
