What happened
The [REDACTED] shell line started up at approximately [REDACTED] [REDACTED]. [REDACTED] who has worked at the Indy Bakery for approximately 9 years and is a senior operator was attempting to show the Team Lead a quality concern with press 6. As she attempted to point to the press she reached underneath the machine guard as the press was activated by the press 6 operator by two hand touch controls. Her right index fingertip distal phalanx was crushed by the press resulting in an amputation.
