Walking

EE was walking west in the expansion adjacent to the supply racks. Video surveillance shows when he reached the first section of the racking he slipped and fell landing awkwardly on his right leg ankle. The area of the floor showed no visible substance or material present to cause such a fall. However when investigating the area the floor was very slippery from an unknown material or substance. Upon further investigation it was discovered that a pallet was in the location [REDACTED] that had a substance on top of it. It was seen on the floor and cleaned right away and the pallet was removed. This substance must have left some kind of a residue that made the floor slippery.
