On [REDACTED] at approximately [REDACTED] I Safety and Security Officer [REDACTED] documented an employee injury report for Marriott Housekeeping employee [REDACTED]. Ms. [REDACTED] was invited into the security office to discuss her injury. Per Ms. [REDACTED] she stated that she incurred her neck injury on [REDACTED] at approximately [REDACTED]. [REDACTED] housekeeping manager was called to assist in documenting Ms. [REDACTED] 's statement. [REDACTED] stated that while working she felt neck pain and believed it was related to stress. [REDACTED] went to seek medical attention and was provided medication. [REDACTED] stated that she still felt neck pain even after taking her medication. Neck pain was felt more while working her normal housekeeping duties. At times pain being at a 10 while making beds. I asked [REDACTED] if she wished to seek medical attention she stated yes. [REDACTED] was provided an authorization for Concentra. I finished documenting her statements and provided her cop
