Before the incident
[REDACTED] was grinding on a die with a horizontal die grinder and a cut-off wheel. She scored and snapped off the setup ear and was going to grind away the rough section left behind.
What happened
As she side-loaded the wheel it disintegrated and chunks struck her in her left forearm and left thigh causing lacerations that required a total of 8 sutures to close. The wheel remnants also struck her face shield and left a gouge out of the shield.
