Before the incident
A maintenance technician was attempting to install a metal access panel. The panel weighs 40 lbs and has two hand-hold cut outs at the bottom of the panel. Small ½ inch diameter fasteners are used to secure the panel in place. The technician had to stand in an awkward position using his left shoulder to hold and balance the panel in position. He placed his right hand through the cut out to help hold the panel while using his left hand to start threading a fastener into position.
What happened
The panel slipped from the technician’s left shoulder falling downward pinching his right thumb between the panel and the machine frame resulting in a laceration injury and required medical treatment.
