The associate was test fitting an auxiliary air knife for the washer. The associate was adjusting the pre-wash guard. As the associate was making the adjustment the associate 's right hand slipped off of the guard and into the drive chain. The associate 's right index finger was caught in the chain due to the lack of following applying the required LOTO for the task. The employee received medical treatment at the ER.
