As the [REDACTED] turned the steering wheel on the forklift to load the conveyor the [REDACTED] felt pain to their left shoulder. Axiom medical was contacted for first aid care. The [REDACTED] requested to be evaluated at the ER despite first aid measurements provided by Axiom. The [REDACTED] was released for full duty by the ER physician
