What happened
Employee was stacking empty (WS [REDACTED]) racks returned from MDO in storage area. While placing the double stacked empty racks on top of the two racks already in place a metal (3lb) tie down bracket came loose from the top rack and fell through the slots in ROP on forklift and struck the employee on top of head.
