Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to amputation hazards created by a lack of training:
(a) On or about 09/30/04 an employee suffered an amputation injury while
operating the
Chicago press brake in the prototype room.
(b) On or about 10/02/04 an employee suffered an amputation injury while
operating the
DR assembly machine in the aluminum vacuum brake division.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to establish an employee training program that ensures that all
employees, both
new hires and current employees, receive the training necessary to ensure
that employees
understand the workings of the machines and equipment they are operating
in
every
department, safety devices on the machines and equipment designed to
protect the
employees, and the potential hazards posed by the machines and equipment
the
employees are operating.
