Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a recognized hazard of electrical shock or burn: (a)M/V Blue Wave: In the lower level tool room, the chop saw was powered by an extension cord with the ground prong missing and the outer insulation was frayed, exposing employees to live parts. Among other methods, one feasible and acceptable method to correct this hazard would be to discard the extension cord and properly replace with an approved cord. (b)M/V Blue Wave: On the Main deck, the port side crane receptacle had reverse polarity when tested with Woodhead electrical tester. This condition exposed employees to possible electrical shock or burns. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to remove and properly rewire the receptacle.
