Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were causing or
likely to cause death or serious physical harm to employees in that
employees were exposed to
lacerations, fractures, and amputations resulting from the following:
a.)M/V Blue Wave, Processing Deck - Where the employer had developed
equipment-specific
lockout/tagout procedures for the control of energy associated with
performing blade changes and
maintenance activities on header machines, the written procedures did not
address controlling and
isolating this energy source when adjusting the conveyor system in the
vicinity of the energized
blade.  On or about 7/7/00, an engineer was observed adjusting the
conveyor at the point of
operation of the energized blade without using alternative protective
measures or a specific
lockout/tagout procedure.
NOTE: Among other methods, one feasible and effective abatement method
would
be to develop
and implement a specific procedure for controlling energy to the header
blade when adjusting the
conveyor at this location.
b.)M/V Blue Wave - The employer had not conducted a periodic inspection of
the energy control
procedures at least annually to ensure that the procedure accurately
reflected the methods being
used and that these procedures were being followed.
NOTE: Among other methods, one feasible and effective abatement method
would be to review
the lockout/tagout procedures annually and verify periodically that they
are being used.
