Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of struck or crushed by the hoisted load and
the crane body of the
Terex T750 Mobile Hydraulic Crane.
a)At the job site, Eckerd Drug Store, Rt 13, Ithaca, New York, on or about
08-
28-2001: An employee was operating a T750 Terex mobile hydraulic crane
with a load that exceeded rated lifting capacities for the length and
position
of the boom in use, causing the crane to over-turn.
Among other methods, feasible and acceptable methods of abatement to
correct these hazards are to:
1.Ensure that a management official complies with the specifications and
limitations applicable to the operation of the crane, specifically to
determine prior to the lift the type of lift and weight of the load to be
lifted, and supplies that information to the designated crane operator,
in accordance with Terex Lifting Operators Manual T750(c)-5-OM,
and Terex Crane Rating Manual.
2.Re-train crane operator employees to determine the weight of the load
prior to making a lift and to determine that the load is within the
rated capacity of the crane, and to ensure that all employees are kept
clear of the suspended load during the lift.
3.Ensure that wood blocks used to support outriggers are strong enough
to prevent crushing, are free from defects, and are of sufficient width
and length to prevent shifting or toppling under load.
Abatement documentation must be submitted for this item.
