Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to electrocution, burns, and bruises:
(a)F/T Alaska Voyager:  There are four circuit breakers missing from the
two
electrical panels on the forward bulkhead in the upper engine room.
One economical and feasible means of abatement would be to install blanks
in the
openings.
(b)F/T Alaska Voyager:  The ground pin has been removed from the plug on
the
portable pressure washer in use in the process area.
One economical and feasible means of abatement would be to replace the plug
with one that is properly grounded.
(c)F/T Alaska Voyager:  The two electrical receptacles by the sinks in the
crews'
head indicated open ground when tested with a Unitest tester.
One economical and feasible method of abatement would be to install ground
fault
circuit interrupter receptacles.
(d)F/T Alaska Voyager:  Electrical receptacles over the bench grinder in
the upper
engine room and over the washing machine in the laundry room indicated
reverse
polarity when tested with a Unitest electrical tester.
One economical and feasible method of abatement would be to remove and
properly rewire the receptacles.
(e)F/T Alaska Voyager:  Faceplate was missing from an electrical
receptacle/light
switch in the bow peak storage room.
One economical and feasible method of abatement would be to replace the
faceplate.
(f)F/T Alaska Voyager:  Fluorescent lights in the bow peak storage are 6
feet 6
inches off the deck and are not guarded to protect the tubes from
accidental
contact and breakage.
One economical and feasible method of abatement would be to install
approved
tube guards.
