Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
(a)Employees in the Disa, Hunter, Osborn, and Maintenance departments were
exposed to the hazard of being struck by loads falling from overhead
hoists, or
by molten metal splashed from ladles suspended from overhead hoists in that
hoists were not inspected and tested on a frequent / daily basis, and
hoists
were not maintained in safe operating condition. Hoists remained in service
with unsafe conditions, including unlabeled pendant control functions,
missing
load hook safety latches, missing capacity markings, and broken speed
selector
switches. Among others, one feasible and acceptable method to correct this
hazard is to implement a hoist inspection program that complies with ANSI
B30.16, the American National Standard for underhung hoists, and to repair
or
remove from service hoists with defects noted during the inspections.
