were exposed to the hazard of receiving various injuries from the lack of
enforcement of the
wearing of seatbelts:
a)On or about June 26, 1998, near the 60 Cycle Electric Test:  An employee
operating a Yale fork truck was not wearing the seat belt.  Employee was
exposed to the hazard of being struck by, or thrown from the fork truck in
an accident.
b)On or about July 8, 1998, near Making Department Line 2:  An employee
operating a Yale fork truck was not wearing the seat belt.  Employee was
exposed to the hazard of being struck by, or thrown from the fork truck in
an accident.
c)On or about July 8, 1998, in the Shipping Department:  An employee
operating a Clark fork truck was not wearing the seat belt.  Employee was
exposed to the hazard of being struck by, or thrown from the fork truck in
an accident.
d)On or about July 8, 1998, in the Shipping Department:  An employee
operating a TCM fork truck was not wearing the seat belt.  Employee was
exposed to the hazard of being struck by, or thrown from the fork truck in
an accident.
e)On or about July 8, 1998, in the Shipping Department:  An employee
operating a Hyster fork truck was not wearing the seat belt.  Employee was
exposed to the hazard of being struck by, or thrown from the fork truck in
an accident.
ABATEMENT NOTES:
Among other methods, one feasible and acceptable method to correct this
hazard is
to require and enforce the use of the seat belts that are provided on the
fork truck
and as required by ASME B56.1-1993, chapter 5.3.19.
