Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of electrocution from accidental contact with exposed live electrical components. The following conditions contributed to the existence of this hazard: (a)M/V Alaska Pioneer, bow storage room: Where employees worked around the energized control panel MD-1, the panel was missing its internal cover plate which exposed live electrical wiring/components. (b)M/V Alaska Pioneer, bow storage room: Access to Panel MD-1 was blocked by numerous items of stored materials. (c)M/V Alaska Pioneer, bow storage room: Where employees worked around an unlabeled energized control panel, the panel was missing its internal cover plate which exposed live electrical wiring/components. (d)M/V Alaska Pioneer, bow storage room: The unlabeled panel contained an unused opening, or 5-inch by 6-inch hole, in its front cover door. (e)M/V Alaska Pioneer, engine room: Access to power control panel 54-S was blocked by clothes and boots hanging in front of and against the panel door. (f)M/V Alaska Pioneer, gear line (alley): An electrical outlet/receptacle indicated reverse polarity when tested. Note: One feasible means of abatement, among others, is to develop a written inspection and maintenance program which would identify potentially hazardous conditions and prevent situations, as those above, from occurring. Until then, the employer should assign a qualified person to replace missing panel covers, clear access to the panels, install a cover plate over the unused opening in the panel, and rewire the receptacle to its proper polarity.
