Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of getting caught in or struck by conveyors: a)Tank Room and Outside Conveyor, On or about 11/19/03: There was no start up alarm for the Kraut Delivery System Conveyor, and the Drag Line. b)Throughout the facility, On or about 11/19/03: Pinch points between conveyor belts and end drums were not guarded. c)Throughout the facility, On or about 11/20/03: Return portions of conveyors were not guarded. Among other methods, one feasible method of abatement is to: 1) Ensure that start up alarms are in place and operational as required per ANSI B20.1 1993, section 5.11.2. 2) Provide guarding for the conveyors as required per ANSI B20.1 1993, sections 5.9 and 6.1. Abatement certification must be submitted for this item.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling from ladders: a)Tank Room, On or about 11/19/03: The top of fiberglass stepladders were used as a step. b)Tank Room, On or about 11/19/03: Fiberglass extension ladders were being used at a near vertical angle. c)Tank Room, On or about 11/19/03: Fiberglass extension ladders were being used so that employees had to climb above the allowable working level. The ladders were not extended. Among other methods, one feasible method of abatement is to: 1) Ensure that ladders are used as per ANSI A14.5 1985, Portable plastic ladders, section 9.3. Abatement certification must be submitted for this item.
