Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads and/or molten metal: In the following locations, overhead cranes and hoists were not maintained in a safe condition: A.Cleaning Room: 1.Three Ingersoll Rand 5 tons hoist were used on the same overhead crane bridge rail. The hoists were not identified. The wire rope on the middle hoist had excessive wear. The wire rope on the end hoist near shipping had excessive wear. 2.Wheelabrator Skip Hoist: Both of the wire ropes provided on the skip hoist showed excessive wear. 3.The lower sheave guard provided on the hoist at the Table Blast Machine was broken, exposing the in-running nip points on one side of the block. B.Basement Sand Mixing Area: The wire rope on the right side of the skip hoist had worn at the bucket connection. A "helper" rope of a smaller diameter had been added using a single wire rope clip. The main hoist ropes on either side were secured to the bucket using only two wire rope clips. C.Large Core Room: The wire rope on the fourth hoist on the crane system had excessive wear. Also, the four hoists were not labeled. D.Mold Line 330. Pinlift: There were five hoists on the crane system which were not labeled. The wire ropes provided on the first three hoists had excessive wear. The hooks provided on the first and third hoists had excessive wear. E.Melt Department: 1.The markings on the pendant control for the Tundish Ladle crane had worn off. 2.The hook provided on the hoist for the 500 pound furnace was partially melted through at the chain connection. Among other methods, one feasible and acceptable abatement method to corect the hazard is: 1.Continue inspecting the cranes and hoists on a yearly basis. 2.Develop and implement a program of monthly, documented, inspections on the cranes and hoists, including but not limited to the wire ropes, hooks and brakes. 3.Perform all needed maintenance and repairs in a timely manner. 4.Maintain all crane/hoist controls markings and provide effective training to all operators in daily inspections and safe operation of cranes.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and/or explosion hazards due to the lack of protective devices on natural gas and/or compressed air piping: A.Cleaning Room: Protective equipment was not provided on natural gas/compressed air torches used in the welding booths to preheat castings. B.Melt Department: Protective equipment was not provided on the natural gas/compressed air torches used to preheat ladels. C.Small Core Room: A Dayton gas fired heater was provided at the stationary core dryer. The gas hose was more than 6 feet long. Among other methods, one feasible and acceptable method to correct the hazard is: 1.Provide protective devices on torch outlets to provide backflow prevention and flash arresting capabilities. 2.Ensure that all stationary gas fired heaters have six feet or less of hose and that shut-off valves are provided at all piping connections. 3.Develop and implement a program of regular inspections on gas hose throughout the shop and replace the hose as needed.

Among other methods, one feasible and acceptable method to correct the hazard is to provide a seatbelt on the forktruck and require that it be used when the truck is being operatd. Abatement Certification is Not Required
