Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the conditions listed below: Lariat Services, Inc., West Ranch 6-30 Rig #2, Fort Stockton, TX. For the period of time up to and including 05/01/06, guywires were not flagged with visible material. API Recommended Practice 4G, Third Edition, April 2004, titled "Recommended Practice for Use and Procedures for Inspection, Maintenance, and Repair of Drilling and Well Servicing Structures, states in Section 14, part 14.5 "The rig contractor shall be responsible for: f. Placing a visible marker on each guywire." Feasible and acceptable abatement methods would include following API RP 4G.

For the period of time up to and including 05/01/06, an auxiliary means of escape, such as a Geronimo line, was not installed prior to personnel working in the derrick. API Recommended Practice 54, Third Edition, April 1999, titled "Recommended Practice for Occupational Safety for Oil and Gas Drilling and Servicing Operations", states in Section 6, part 6.10.1 "On all land rigs, the derrick or mast shall have an auxiliary means of escape installed prior to personnel working in the derrick" Feasible and acceptable abatement methods would include: Ensure an auxiliary means of escape, such as a Geronimo line is installed prior to personnel working in the derrick.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the conditions listed below: Lariat Services, Inc., West Ranch 6-30 Rig #2, Fort Stockton, TX. For the period of time up to and including 05/01/06, the rig had not been rigged up in a safe condition in the following instances: a) The guy wires stabilizing the derrick had only one fist-clamp in place. b) High pressure mud hoses were not secured in place. c) Possible sources of ignition were not removed from the drill floor / top dog house area. API Recommended Practice 54, Third Edition, April 1999, titled "Recommended Practice for Occupational Safety for Oil and Gas Drilling and Servicing Operations", states in Section 6, part 6.3.2 "Well operations involving the rig shall not be commenced until the rig is rigged up in a safe manner." Feasible and acceptable abatement methods would include: Ensure three or more fist clamps are used to secure the guy wires. Ensure all high pressure hoses are secured in place. Ensure all possible sources of ignition are removed from the drill floor / top dog house area.
