Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to severe laceration and amputation hazards: (a)F/V Alaska Patriot, Factory Deck: On or about February 2, 2008 and at times prior thereto, energy isolation procedures (lockout/tagout, LO/TO) had not been initiated when performing blade sharpening on the Tokai Header model number PHC-550-R, machine number 103. The employee was exposed to severe laceration and amputation hazards which would require hospitalization. Among other methods, one feasible and acceptable abatement method to correct the hazard is to follow the lockout/tagout program the employer had developed.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: (a)F/V Alaska Patriot, Weather deck, Stern, Port-side: On or about February 2, 2008 and at times prior thereto, an employee walked into the "bight" of a mooring line routed around the port-side stern bollard. The employee was exposed to struck-by hazards in the event the mooring line slacked, slipped off the bollard, and snapped back in the direction of the employee. Among other methods, one feasible and acceptable abatement method to correct the hazard is to re-train employees in line-handling hazard recognition and vessel mooring requirements.
