a)BEQ 13 at the Northeast Corner: The Komatsu 25 Forklift was left
unattended at the
construction site with the load engaging means fully raised. The half
pallet of concrete
block on the forks was partially resting on a supported scaffold, exposing
employees to
crushing hazards and/or fall hazards in the event of a hydraulics failure.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to follow by ASME/ANSI Standard B56.1-2000 Section 5.2.11(a)(4) Before
leaving the
operator's position: lower load-engaging means fully, unless supporting an
elevated
platform.
