that, an employee
was exposed to crushing hazards while working/walking next to loaded
forklifts:
a) On or about December 12, 2006, and at times prior, employees were
exposed to crushing
hazards while working/walking next to loaded forklifts in that:
1) Forklifts are not provided with designated aisle-ways.
2) Employees and Supervisors are not properly trained on the safe
operation of
forklifts.
Among other methods, one feasible and acceptable abatement method to
correct this
hazardous condition would be develop and implement forklift training as
required by the
American National Standards Institute (ANSI) B56.1-1969 "Safety Standards
for Powered
Industrial Trucks" section 6, and as recommended by the forklift
manufacture.
Abatement Note:  Abatement certification and documentation is required for
this item
(see enclosed "Sample Abatement-Certification Letter").
