Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, an employee was exposed to crushing hazards while operating a forklift he had not properly trained on: a) On or about November 16, 2007 and times prior, employees were exposed to crushing hazards while operating forklifts in that employees are not properly trained on the safe operation of the newer forklifts. Among other methods, one feasible and acceptable abatement method to correct this hazardous condition would be develop and implement forklift training as required by the American National Standards Institute (ANSI) B56.1-1969 "Safety Standards for Powered Industrial Trucks" section 6, and as recommended by the forklift manufacture. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, the yard where the forklifts travel is not level (uneven surfaces), exposing employees to crushing injuries in the event of a forklift tip-over or other accident: a) On or about November 16, 2007 and times prior, employees operating forklifts were exposed to crushing injuries. There were uneven surfaces in the yard that a forklift could easily travel over because of their location and obscurity. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to develop a reporting/audit system to search out this conditions, then install temporary protection (i.e. covers, barricades, etc.) until a permanent fix can be installed.. Abatement Verification: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification

Among other methods, one feasible and acceptable abatement method to correct this hazard is to implement a preventative maintenance program in accordance with the manufactures recommendations. Abatement Verification: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification
