Triangle Conveyor Pit - On or about 12/23/2005, a lineman was injured when
he
attempted to break his fall in the stairway along the conveyor. His left
arm went through
an opening formed by a belt, chain guard and the screen support surface.
The headbar
that was attached to a screen cycled through, catching the his arm in the
opening and
shearing it off when the headbar met the edge of the screen support
surface.
