Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to 'caught in' hazards: Triangle Conveyor Pit - On or about 12/23/2005, a lineman was injured when he attempted to break his fall in the stairway along the conveyor. His left arm went through an opening formed by a belt, chain guard and the screen support surface. The headbar that was attached to a screen cycled through, catching the his arm in the opening and shearing it off when the headbar met the edge of the screen support surface.
