Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were potentially exposed to fire and explosion hazards from the ignition of potential natural gas leaks due to the lack of a readily accessible manual shut-off valves and improperly supported natural gas piping:: A.) Dirty Side /Area Near the Electrical Panel Enclosed By A Cage: A readily accessible manual shut-off valve was not installed on the natural gas piping within at least 6-feet of the natural gas fired space heater. The existing shut-off required tools to be operated. B.) Area Near the Checker Shack: A readily accessible manual shut-off valve was not installed on the natural gas piping within at least 6-feet of the natural gas fired space heater. The existing shut-off required tools to be operated. In addition, the natural gas piping that led from the heater, across the floor, and up the wall was not adequately supported/restrained. ABATEMENT NOTE: Among other methods, feasible and acceptable means to correct the condition include complying with the applicable requirements outlined in the National Fuel Gas Code (NFPA 54/ANSI Z223.1 AND/OR the International Fuel Gas Code. Shutoff Valves: National Fuel Gas Code (NFPA 54/ANSI Z223.1) Sections 5.12 shutoff valves shall be selected given consideration to pressure drop, service involved, emergency use, and reliability of operation. National Fuel Gas Code (NFPA 54/ANSI Z223.1) AND/OR the International Fuel Gas Code requires the manual shut-off valve to be separate from the equipment served, readily accessible, and located at least within 6-feet of the equipment served. Pipe Support, Hangers, and Anchors: National Fuel Gas Code (NFPA 54/ANSI Z223.1) Sections 7.2.6.1 (a) Piping shall be supported with hooks, brackets, straps, bands, or hangers suitable for the size of the piping, of adequate strength/quality, and located at intervals so as to prevent or damp out excessive vibration. Piping hsall be anchored to prevent undo strain on connected equipment. (b) Spacing of supports shall not be greater than 6-foot to 10-foot intervals (Based on pipe diameter and Table 7.2.6)

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were potentially exposed to blunt force trauma injuries from dropped, falling, and toppling crane loads in that care was not taken to ensure the load did not contact any obstructions and there was no sudden acceleration or deceleration of the moving load [in accordance with ANSI B30.11 Monorail Cranes Section 11-3.2.3 (c)(1)-(2)]: A.) Young Galvanizing, Inc.- Pulaski, PA-Crane Program/Quench Tank Area: The crane's load was knocked against the quench tank to dislodge excess galvanizing material. ABATEMENT NOTE: Among other methods, feasible and acceptable means to correct the condition include training the operators in accordance with the requirements noted in ANSI B30.11 Monorail Cranes Section 11-3.2.3 (c)(1)-(2) which states - care will be taken to ensure that the load does not contact any obstructions and there is no sudden acceleration or deceleration of the moving load.g
