Occupational Safety and Health Act of 1970, Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  A.)  Young Galvanizing, Inc., 8281 Mercer Street, Pulaski, PA  16143, on or about September 30, 2020:    Employees were exposed to struck-by hazards from falling loads during monorail crane rigging operations.  Two crane operators were rigging a lift using two separate monorail cranes.  One of the crane operators operated the hoist of monorail crane #4 in the upward position to remove slack in the hoist.  The upward motion upset the load and the load fell over onto the legs of the crane operator who was rigging monorail crane #5.  A designated person was not involved in the pre-lift activities of the load including, but not limited to, analyzing the lift and load to be lifted, crane operator coordination, condition on the rigging, and safe rigging of the load.    B.)  Young Galvanizing, Inc., 8281 Mercer Street, Pulaski, PA  16143, on or about September 30, 2020:  Employees were exposed to struck-by hazards from falling loads during monorail crane rigging operations.  Two crane operators were rigging a lift using two separate monorail cranes.  One of the crane operators operated the hoist of monorail crane #4 in the upward position to remove slack in the hoist.  The upward motion upset the load and the load fell over onto the legs of the crane operator who was rigging monorail crane #5.  The crane operator operating monorail crane #4 did not verify that the area around the load was cleared of personnel before operating the hoist of monorail crane #4.
