Occupational Safety and Health Act of 1970, Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: A.) Young Galvanizing, Inc., 8281 Mercer Street, Pulaski, PA 16143, on or about September 30, 2020: Employees were exposed to struck-by hazards from falling loads during monorail crane rigging operations. Two crane operators were rigging a lift using two separate monorail cranes. One of the crane operators operated the hoist of monorail crane #4 in the upward position to remove slack in the hoist. The upward motion upset the load and the load fell over onto the legs of the crane operator who was rigging monorail crane #5. A designated person was not involved in the pre-lift activities of the load including, but not limited to, analyzing the lift and load to be lifted, crane operator coordination, condition on the rigging, and safe rigging of the load. B.) Young Galvanizing, Inc., 8281 Mercer Street, Pulaski, PA 16143, on or about September 30, 2020: Employees were exposed to struck-by hazards from falling loads during monorail crane rigging operations. Two crane operators were rigging a lift using two separate monorail cranes. One of the crane operators operated the hoist of monorail crane #4 in the upward position to remove slack in the hoist. The upward motion upset the load and the load fell over onto the legs of the crane operator who was rigging monorail crane #5. The crane operator operating monorail crane #4 did not verify that the area around the load was cleared of personnel before operating the hoist of monorail crane #4.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: A.) Young Galvanizing, Inc., 8281 Mercer Street, Pulaski, PA 16143: An employee was hospitalized following a workplace injury on September 30, 2020. The hospitalization was not reported to the OSHA Erie Area Office within 24-hours of the hospitalization. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
