Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that employees
were exposed to crushing and/or struck-by injuries related to hydraulic
press operation.
a)  Continental Structural Plastics, Inc. - Carey, Ohio:  On or about
February 11, 2009, the
employer did not ensure that fastening components and methods were
sufficient to hold the upper
die in the clamped position during the complete slide stroke of the press
and did not consider
all forces that may be applied to the clamps.  During normal operation of
the Williams-White
hydraulic power press #15 employees were exposed to crushing and struck-by
injuries when
fastening components failed to keep the bolster or upper die/mold attached
to the upper moving
portion of the press.
Abatement Note: Among other methods, one feasible and acceptable abatement
method to
correct this hazard is to comply with American National Standard Institute
(ANSI) B11.2-1995
"Hydraulic Power Presses - Safety Requirements for Construction, Care, and
Use."
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
written records.
