Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer
did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to crane and hoist components that had not been
incorporated into a
frequent inspection program:
a)  Gasbarre Products, Inc. - On or about 10/14/10, frequent inspections
(i.e. daily to weekly
to monthly inspections) were not being performed on the components
associated with the
cranes and hoists.  Only annual inspections were performed by an outside
contractor.  These
cranes/hoists were located in areas that included but were not limited to
A-Bay, B-Bay, C-
Bay, D-Bay, and E-Bay.
ABATEMENT NOTE:
Among other methods, feasible and acceptable means to correct the
condition include but are
not limited to:
Per ANSI B30.16 and ANSI B30.11 Frequent Inspections are conducted on a
daily to weekly
to monthly intervals depending on the use of the equipment as follows:
Normal service - monthly
Heavy service - weekly to monthly
Severe service - daily to weekly
Develop a frequent inspection schedule based on the manufacturers manuals,
service
conditions, and the requirements noted in ASME B30.16 - Hoists, ASME
B30.11 - Monorail
and Underhung, and ASME B30.10 - Hooks
A Qualified Person Shall Determine Whether Conditions Found During The
Inspection
Constitute A Hazard And Require A More Detailed Inspection. Adjustments,
Repairs, Or
Replacements Shall Be Made, As Necessary, In Accordance With ASME B30.11,
ASME
B30.16, Manufacturers Manual Deficiencies Discovered During This
Inspection Shall Be
Reported Immediately To The Supervisor Or A Designated Person.
At a Minimum Frequent Inspections Must Include a Review of:
-Operating Mechanisms For Proper Operation, Proper Adjustment, And Unusual
Sounds; Such As, But not Limited To, Squeaking, Grinding, Grating, Etc.
-Hoist Upper Limit Device Of Electric- Or Air Powered Hoists, Without A
Load On
The Hook At The Beginning Of Each Shift. Care Shall Be Exercised. The Load
Block
shall Be Inched Into Its Limit Device Or Run In At Slow Speed On
Multi-Speed Or
Variable-Speed Hoists.
-Hoist Braking System For Proper Operation.
-Tanks, Valves, Pumps, Lines, And Other Parts Of air Or Hydraulic Systems
ForLeakage
-Hooks And Latches, In Accordance With Asme B30.10
-Hook Attachment And Securing Means
-Warning Device(S) For Proper Operation
-Chain Or Wire Rope In Accordance With Asme B30.16
-Hoist Load Chain In Accordance With Para. 16-2.5.1 Or 16-2.6.1.
-Rope Or Load Chain Reeving For Compliance With Recommendations Of The
Hoist
Manufacturer.
-Placement Of End Stops
ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED:  Abatement
certification and documentation required within 10 days after abatement
date.  The
certification shall include a statement that abatement is complete, the
date and method
of abatement, and state that employees and their representatives were
informed of the
abatement.  Abatement documentation shall include documents demonstration
that
abatement is complete, such as evidence of the purchase or repair of
equipment,
photographic or video evidence of abatement or other written records.

employees were exposed to crane/hoist control pendants that had not been
marked as to the
intended use of each button:
a)  Gasbarre Products, Inc. --- Cranes and Hoists --- On or about
10/14/10, pendant control
boxes were not marked to indicate the function of the control buttons.
The pendants for
these cranes/hoists were located in areas that included but were not
limited to A-Bay, B-Bay,
C-Bay, D-Bay, and E-Bay.
ABATEMENT NOTE:
Among other methods, feasible and acceptable means to correct the
condition include but are
not limited to:
1)  Complying with the applicable requirements noted in ASME B30.11
Monorails and
Underhung Cranes and/or ASME B30.16 - Hoists
2)  Replace missing or illegible function labels on pendant control
stations.
3)  Label markings should be standardized throughout the facility and/or
work area per
ASME B30.11 and ASME B30.16.
