Gasbarre Products, Inc.; on or about 5/5/2022 and at times prior thereto, Bay 4 West side exit route had flammables stored in the exit route. Containers of flammable and combustible materials including but not limited to gasoline, paints, and oils were stored on the floor outside of flammable storage cabinets.  There was no space remaining within the flammable cabinets, so employees used the area around the cabinet for storage of additional containers of flammable and combustible liquids.

Employees were exposed to slip trip and fall hazards while walking through the area.  b.) Gasbarre Products, Inc.; on or about 5/5/2022 and at times prior thereto, Isle ways in bay 4 were obstructed by debris and other material exposing employees to potential trip hazards while walking through bay 4 of the shop.

Gasbarre Products, Inc.; on or about 5/5/2022 and at times prior thereto, Safety signage storage area-  One compressed N.O.S. gas cylinder was free standing and not secured while in storage next to the safety sign storage area during the inspection.   c.) Gasbarre Products, Inc.; on or about 5/5/2022 and at times prior thereto, Bay 1 North side area- One compressed N.O.S. gas cylinder was free standing and not secured while in storage next to a welder during the inspection.

Gasbarre Products, Inc.; on or about 5/5/2022 and at times prior thereto, Employees periodically operated a Hyster 70 forklift that had a damaged seatbelt system which prevented use of the seatbelt.  b.) Gasbarre Products, Inc. Bay 2 area; on or about 5/5/2022 and at times prior thereto, employees operate a Hyster 70 propane forklift in various areas of the facility throughout each work shift. The tires on the powered industrial truck were damaged in that they were missing large chunks of tire material.
