A workbench, painting products, supplies, and waste cans were stored within one foot of the spray booth.

Gasbarre Products, Inc., 590 Division Street, Dubois, Pa 15810, Large Bay doors in Devilbiss brand Spray booth, on or about March 13, 2014 and times prior thereto: Deposits of combustible residues from over-spray had accumulated on the interior walls of the spray booth. The booth had not been cleaned for approximately three weeks prior to March 13, 2014.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) was not established and implemented with worksite specific procedures for required respirator use: a.) Gasbarre Products, Inc., 590 Division Street, Dubois, Pa 15801,-Devilbiss spray paint booth- on or about January 9th, 2014 and times prior thereto: Company employees routinely use 3M brand full facepiece air purifying respirators and North brand half mask air purifying respirators with organic vapor cartridges. The company's written Respiratory Protection Program did not include site specific information including, but not limited to, the following: 1.) types of respirators and air purifying cartridges in use at the company, 2.) procedures and / or schedule for replacement of respirator cartridges, 3.) the required completion of Appendix C to Section 29 CFR 1910.134: OSHA Respirator Medical Evaluation Questionnaire, 4.) fit testing procedures, 5.) a description of training procedures, 6.) procedures and the schedule for cleaning and disinfecting respirators, 7.) procedures for regularly evaluating the effectiveness of the respiratory protection program.

A change out schedule for replacing the cartridges had not been developed nor incorporated into the company's Respiratory protection program.
