Lariat Services, Inc., Rig 31, University 32-18 #4H, Ward County, TX.  For
the period of time
up to and including 08/14/08, the employer did not ensure that the
stairways used by employees
for access to and from the mud pits, had bolts in place on the screws that
were to hold the
stairways in place.
American Petroleum Institute states in Recommended Practice for
Occupational
Safety for Oil
and Gas Well Drilling and Servicing Operations, API Recommended Practice
54, Third Edition,
August 1999, Reaffirmed March 2007, states in paragraph 9.3.8 "All open
stairways of four (4)
or more risers shall be securely fastened and equipped with handrails and
midrails extending the
entire length of the stairway."
Feasible and acceptable methods of abatement include ensuring all
handrails and midrails extend
the entire length of the stairway.

Table 2 and Table 3 for minimum number of clips, and torque required."

of Double Saddle Clips, indicate a minimum of 3 clips for wire rope
diameter 3/4-inch.
