Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to hazards while working at elevated heights during the change-out of the bulk wood chip bag. (a)Main Production Room; The employee worked at different heights from a pallet which was supported by the forks of a powered industrial truck in order change- out the bulk wood chip bag. Adequate procedures were not implemented to prevent the employee from falls to the concrete floor below. Among other methods, one feasible and acceptable abatement methods to correct this hazard is to follow the guidelines in the American National Standard Institute / Industrial Truck Standards Development Foundation (ANSI/ITSDF) B56.1-2005, Safety Standard for Low Lift and High Lift Trucks, Section 4.17 Elevating Personnel. Abatement certification and documentation are required for this item.
