(a)In the shaded break area on the north side of the museum, the 14 inch
diameter
abrasive wheel on a Milwaukee Heavy Duty 14 inch cut off saw, catalog
number
6176-20, serial number 968F910191257, was used to grind down the diameter
of
a hammer drill bit.  The end of the bit was too large to fit in the hammer
drill's
chuck.  Employees lifted the moveable guard on the saw, held the bit in
their
hands, and used the side of the abrasive wheel to grind down the diameter
of the
bit.  Employees were exposed to the hazard of being struck by the wheel.
Among other methods, one feasible and acceptable method to correct this
hazard
is to follow the instructions in the Operator's Manual for the Milwaukee
Heavy
Duty 14 inch cut off saw, catalog number 6176-20, serial number
968F910191257.  On page 3 the manual states in part: "SPECIAL SAFETY
INSTRUCTIONS - ABRASIVE CUT-OFF MACHINES, Wheel Instructions, 1.
Use only the edge (not the sides) of the wheel for cutting.  Machine
Instructions,
9. Always keep the guard in place.  11.  Never make any free hand cuts.
Always
place the workpiece between the vise and fence when making cuts."
