employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to fall hazards:
On or about January 12, 2012, at the worksite located at or near 8 Bruce
Street
in Morrilton, Arkansas, south front entrance to building, filler blocks
were not
inserted between the rungs on the approximately 8 ft. wooden job-made
ladder
being used by employee on the front entrance of the building.  Employee was
exposed to an approximately 4 ft. fall from ladder to ground level below.
