29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a) Jobsite:  On April 21 2011, and times prior thereto, employees were using a scaffold that was not fully planked to stand on to wash down the newly constructed exterior brick walls on the outside of the building. 29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) Jobsite: On April 21 2011, and times prior thereto, an employee was standing on a scaffold near the edge of the roof exposed to a fall to a lower level of approximately 20 feet. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Jobsite Roof: On April 21 2011, and times prior thereto, employees were working on a low slope roof at a height of approximately 18 feet without  a guard rail, safety net or personal fall arrest system in place.
