(a) 635 N Park Ave, Idaho Falls, ID 83402: On or about April 23, 2019 and at times prior, the employee driving the TEREX HANDLERS: model TH10560, Serial #045282 powered industrial truck for moving building material on the jobsite, had not been wearing his seatbelt. Note: Abatement certification is required for this item.

29 CFR 1926.1153(c)(1): Specified exposure control methods. For each employee engaged in a task identified on Table 1, the employer shall fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section. (a) 635 N Park Ave, Idaho Falls, ID 83402: On or about April 23, 2019 and at times prior, a Diamond Products CC800M block saw being used to cut cinder blocks had cracked and non-fitting water lines which greatly diminished the silica dust dampening properties of the saw. Note: Abatement certification is NOT required for this item.
