29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(IDENTIFY MACHINERY OR EQUIPMENT)(THIS INCLUDES ENFORCEMENT OF PROCEDURES)      a)  Creative Foam Composite Systems, Inc., 1800 Pike Road, Longmont, CO:  The employer had not developed and implemented specific energy control procedures pertaining to the 5-Axis mill machine. Condition exposed employees to hazards associated with unexpected startup and/or release of stored energy while servicing and/or performing maintenance.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a)  Creative Foam Composit Systems, 1800 Pike Road, Longmont, CO:  The employer did not ensure that periodic inspections of the employers energy control procedures for the Anderson 5-Axis Mill machine were done on an annual basis.    Abatement note: The following text was taken from 29 CFR 1910.147 (6) to assist the employer is compliance with the provision:   6)  Periodic inspections:  [A] The periodic inspection shall be performed by an authorized employee other than the one(s) utilizing the energy control procedure being inspected.   [B] The periodic inspection shall be conducted to correct any deviations or inadequacies identified.   [C] Where lockout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.   [D] Where tagout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized and affected employee, of that employee's responsibilities under the energy control procedure being inspected, and the elements set forth in paragraph (c)(7)(ii) of this section.   (ii) The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Creative Foam Composit Systems, 1800 Pike Road, Longmont, CO:  Located on the 5-Axis mill machine, the rotating cutting tool was not guarded to prevent accidental contact.  Condition exposed employees to cuts and or lacerations.

29 CFR 1910.213(c)(1):  Each circular hand-fed table saw was not guarded by a hood that completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut.     a)  Creative Foam Composite Systems Inc., 1800 Pike Road, Longmont, CO:  Located on the Grizzly brand (G0451) sliding table saw, the guard did not completely enclose the portion of the blade above the table.  Condition exposed employees to inadvertent contact with the rotating blade.     Abatement Note: The following information is provided to assist the employer with compliance regarding table saw hoods.  The hood and mounting shall be arranged so that the hood will automatically adjust itself to the thickness of and remain in contact with the material being cut but it shall not offer any considerable resistance to insertion of material to saw or to passage of the material being sawed. The hood shall be made of adequate strength to resist blows and strains incidental to reasonable operation, adjusting, and handling, and shall be so designed as to protect the operator from flying splinters and broken sawteeth. It shall be made of material that is soft enough so that it will be unlikely to cause tooth breakage. The hood shall be so mounted as to insurethat its operation will be positive, reliable, and in true alignment with the saw; and the mounting shall be adequate in strength to resist anyreasonable side thrust or other force tending to throw it out of line.

29 CFR 1910.147(c)(2)(iii):  New machines and equipment that were installed after January 2, 1990 were not provided with energy isolating devices designed to accept lockout devices:        (a)  Creative Foam Composit Systems, 1800 Pike Road, Longmont, CO:  The employer had not ensured that the primary air supply valve on the Anderson 5-Axis Mill machine installed after 2009 was capable of accepting a lock. Condition exposed employees to accidental release of compressed air.
