29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: (a) Creative Foam Composite Systems, LLC, 2221 Clayton Place, Berthoud, CO 80513: On and before October 31, 2016, the employer did not ensure that adequate housekeeping procedures were in place to prevent the accumulation of combustible balsa wood dust and composite foam dust in the CNC production area. The employer allowed combustible dust to accumulate where cutting and shaping operations occurred. This condition exposed employees to fire hazards.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section: (a) Creative Foam Composite Systems, LLC, 2221 Clayton Place, Berthoud, CO 80513: On and before October 31, 2016, the employer did not develop, document, and utilize specific procedures for the control of potentially hazardous energy when cleaning and performing maintenance on machines such as, but not limited to, the CNC DMS/Fagor 8055. Employees entered into the DMS Fagor 8055 to clean and to change out blades when they did not have exclusive control of the machine, and when interlocking doors were not in use. This machine was not locked out and tagged out during these processes. This condition exposed employees to amputation hazards.

The employer operated machines using multiple energy sources, such as, but not limited to, the CNC DMS/Fagor 8055. This condition exposed employees to amputation hazards.
