29 CFR 1910.212(a)(3)(ii):  Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a)Creative Foam Composite Systems, LLC. @ 2221 Clayton Place, Berthoud, CO 80513: On and before April 7th, 2015, the employer did not ensure that the RYOBI Router Table, located in the CNC Validation Department to process Heel Kit Foam Parts was adequately guarded. Employees were required to hold and feed Small Heel Kit Foam Parts (HOI- 49) into the machine point-of-operation without the safety guard in place during the operating cycle(s). This condition exposed employees to amputation hazards.  On April 7th, 2015, an employee holding and guiding the Small HOI-49 Foam Part into the unguarded rotating blade of the router suffered a right hand, middle finger tip, amputation.
