29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:    (a) Creative Foam Composite Systems, LLC at 2221 Clayton Pl. Berthoud, CO 80513: On or before 11/5/18 the employer did not protect employees from injury in that the emergency stop button on the Anderson Model NC1325IP/PT, SN# FAANCIP96131 and Anderson EXXACT PLUS DUO, SN# FANNCEX92041 was used in lieu of performing its energy control procedure prior to cleaning. This condition exposed employees to hazardous energy.
