29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, or was secured: a) Reyes Construction LLC at 2224 Spruce Street, Boulder, CO 80302: On or before 6/18/11, the employer did not ensure that a ladder used to gain access to the roof had its side rails extended up 3 rungs or was secured. Condition exposed employees to fall hazards of 8'. Abatement note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").

29 CFR 1926.404(b)(1)(ii): All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which were not a part of the permanent wiring of the building or structure and which were in use by employees, did not have approved ground-fault circuit interrupters for personnel protection: a) Reyes Construction LLC at 2224 Spruce Street, Boulder, CO 80302: On or before 6/18/11, the employer did not ensure that the electrical circuit used to power the 120 volt Skilsaw had a GFCI. The Skilsaw, model 77, was not double insulated, but was grounded, and was used in dry conditions. Condition exposed employees to electrical shock hazards. Abatement note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").
