OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:   On or about November 20, 2024, at a job site located at 9051 South Chicago Road, Oak Creek, WI 53154, an employee was exposed to a fall hazard of approximately 11 feet while using the quick fold lid of an Equipter RB4000, as a means of access and to install underlayment on an existing residential roof.

(a) On or about November 20, 2024, employees conducting roofing work on an existing residential roof with a 8:12 pitch, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 15 feet to the ground below.  (b) On or about November 20, 2024, employees conducting roofing work on the lower entryway roof, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 10 feet to the ground below.
