29 CFR 1910.178(a)(6): The user did not see that all nameplates and markings were in place and were maintained in a legible condition:\r\n\r\na) Warehouse Area-  The employer failed to ensure that all powered industrial trucks, such as the Caterpillar Forklift #FL129 and Caterpillar Forklift #FL154, were provided with nameplates.  \r\n\r\nIn accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(o)(5): The load engaging means of the industrial truck were not placed under the load as far as possible when moving a load:\r\n\r\na) Dock Area- The employer failed to ensure that all powered industrial trucks lifting loads used the load engaging means, or forks, of the truck to lift loads and placed them fully under the load being moved.  Forklifts were found to be using methods, such as straps with metal hooks and clips, other than the load engaging means to move loads. \r\n\r\nIn accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(p)(1): Powered industrial trucks found to be in need of repair, defective, or in any way unsafe, were not taken out of service until it has been restored to safe operating condition:\\\\r\\\\n\\\\r\\\\na) Warehouse & Dock Areas- The employer failed to ensure that all powered industrial in need of service were removed from service until repairs could be made.  Powered industrial trucks that were found to have unsafe conditions included; Toyota Forklift #FL218 that had a bad rear right  and front right tires, Caterpillar Forklift #FL154 that had bad rear right and rear left tires, Toyota Forklift #FL202 that had a bad rear right tire, and Caterpillar Forklift #FL143 with a bad rear left tire. \\r\\n\\r\\nIntegrated Airline Services, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.178(p)(1), which was contained in OSHA Inspections Number 314556788, Citation 1, Item 3, issued on August 4, 2011, with respect to the workplace at 11301 Irving Park Road, Franklin Park, Illinois.  .\\r\\n\\r\\n\\\\r\\\\n\\\\r\\\\n\\\\r\\\\nIn accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
