29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, rotating, or falling. a) Warehouse - The LP gas cylinder located on the back side of the gas sit down type (fork lift) power industrial vehicle was not properly secured. Hyster ID #90 & 150, SN # A024D04715B. The bracket on the left side of the cylinder was unsecured. b) Warehouse - The LP gas cylinder located on the back side of the Toyota gas sit down type (fork lift) power industrial vehicle was not properly secured. ID #12, SN # FGC35. The brackets on the left and right side of the cylinder were not connected. c) Warehouse - The LP gas cylinder located on the back side of the Caterpillar gas sit down type (fork lift) power industrial vehicle was not properly secured. ID #15, SN # AT81F00438. The bracket on the left side of the cylinder was missing and not connected.

Warehouse - Powered industrial vehicles were found to be in need of repair, defective, and unsafe were not taken out of service until they were restored to safe operating condition such as the: a) Hyster #90/150; SN # A024D04715B, Headlights were not working. b) Nissan #7 SN #CUJ02A25PV; Headlights were not working. c) Hyster # 19 SN #D187V26985Z; Headlights were not working. d) Toyota #12 SN#FGC35; Headlights were not working. e) Komatsu #60 SN#360689A;Back up alert and headlights were not working. f) Caterpillar #23 AT81D00276; Headlights were not working. g) Toyota #16 Headlights were not working, and the strobe light was not working.
