29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard:   (a)  In the warehouse in front of the office:  On July 15, 2013 and at times prior to; the employer failed to maintain passageways without obstruction in that 10 steel bolts protruded above the concrete surface approximately 1/4 inch creating a trip hazard.    (b)  In the center of the warehouse:  On July 15, 2013 and at times prior to; the employer failed to maintain passageways without obstruction in that 5 electrical extension cords used to provide service for fans, were laying on top of the concrete surface creating a trip hazard.
