The Caterpillar Forklift #FL143 was found to have the unsafe condition of a missing side panel.

29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating, or falling:      a) Warehouse - The LP gas cylinder located on the back side of the gas sit down type (fork lift) power industrial vehicle was not properly secured.  Caterpillar FL 155, model #P5000, serial #AT3511113.  The bracket on the right side of the cylinder was unsecured.        b) Warehouse - The LP gas cylinder located on the back side of the gas sit down type (fork lift) power industrial vehicle was not properly secured.  Caterpillar FL 127, model #P5000, serial #AT3504570.  The bracket on the left side of the cylinder was unsecured.      c) Warehouse - The LP gas cylinder located on the back side of the gas sit down type (fork lift) power industrial vehicle was not properly secured.  Caterpillar FL 141, model #P5000, serial #AT3506715.  The bracket on the left side of the cylinder was unsecured.     d) Warehouse - The LP gas cylinder located on the back side of the gas sit down type (fork lift) power industrial vehicle was not properly secured.  Caterpillar FL 130, model #P5000, serial #AT3504444.  The bracket on the left side of the cylinder was unsecured.     e) Warehouse - The LP gas cylinder located on the back side of the gas sit down type (fork lift) power industrial vehicle was not properly secured.  Caterpillar FL 137, model #P5000, serial #AT3505026.  The bracket on the left side of the cylinder was unsecured.

Powered industrial trucks that were found to have unsafe conditions included; Toyota #FL 204 that had a bad rear right tire, Caterpillar Forklift #FL127 that had a bad front right and left tires, Toyota #FL 272 that had a bad front right and left tires, Toyota #FL 218 that had a bad front left tire, Toyota #FL 216 that had a bad front right and left tires.     Integrated Airline Services, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.178(p)(1), which was contained in OSHA Inspections Number 463138, Citation 2, Item 1, issued on July 24, 2012, with respect to the workplace at 11301 Irving Park Road, Franklin Park, Illinois.     Integrated Airline Services, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.178(p)(1), which was contained in OSHA Inspections Number 77278, Citation 2, Item 1, issued on August 22, 2011, with respect to the workplace at 11301 Irving Park Road, Franklin Park, Illinois.
